is a modified, 3-item version of the AUDIT instrument to identify persons who are hazardous drinkers or have active alcohol use disorders.

The CRAFFT Screening Interview
is a behavioral health screening tool for use with children ages 12-18 and is recommended by the American Academy of Pediatrics' Committee on Substance Abuse with adolescents.

Assist V 3.0
consists of eight questions covering tobacco, alcohol, cannabis, cocaine, amphetamine-type stimulants (including ecstasy) inhalants, sedatives, hallucinogens, opioids and 'other drugs'.

DAST
is a 28-item self-report scale that consists of items that parallel those of the Michigan Alcoholism Screening Test (MAST). The DAST has “exhibited valid psychometric properties” and has been found to be “a sensitive screening instrument for the abuse of drugs other than alcohol.

SBIRT Screening Questionnaire
is a useful screening tool which includes both the Audit and CRAFTT screens for both adults and adolescents.

is a checklist to record the type of referral to treatment for a patient.
